Supernumerary teeth (STs) are extra teeth that exceed the regular number. Supernumerary teeth are extremely rare in front of the mandible. Considering all reported STs in this region were conical-shaped, this report aimed to present an exceptional case of a molariform ST in the mandibular incisor region and its long-term consequences. The fact that the case was recorded at two different ages adds to its uniqueness. At the age of 10, when the patient underwent surgical extraction of the impacted molariform ST and retained central incisor. The same patient, aged 25, returned with complaints of crowding and a midline shift of the mandible to the right, periodontal problems, and cosmetic issues. This case report highlights the importance of early detection and appropriate multidisciplinary treatment of ST to avoid or minimize the consequences, the correction of which later requires significant professional effort and financial means.
